Master Plan for the Peninsula Wellness Community Hub: an initiative of the Peninsula Health Care District

The Master Plan for a health and wellness “hub” represented a transformative initiative aimed at enhancing the Peninsula Health Care District’s older residents’ health and well-being. This comprehensive project involved analyzing demographics, assessing the district’s health needs, evaluating existing services, identifying gaps, and engaging stakeholders to develop a dynamic wellness hub that goes beyond traditional community centers.

Our focus was on intergenerational engagement, addressing the specific needs of older adults while also involving secondary audiences to create a holistic approach to community health. Through evidence-based Experience and Service Design, we adopted user-centered methods to craft and improve services, optimize operational models, and develop physical infrastructure. The goal was to design and prototype tailored services and programs to foster a thriving, interconnected wellness ecosystem that supports residents’ well-being throughout their lives. This document summarizes the collaborative work completed during the project's foundational phase.
